Members agedhorse
Are you tinning the wires? If you are, don't. That's the most common cause.

Members WynnD
Setting the strain relief correctly is important. If that is loose, the wires will tug out pretty easily.
Members Tomm Williams
Are the threads in the set screws backing out ?
Members Unalaska
Make sure the cables are wrapped correctly, if not then the conductors can get twisted and pull one right out over time. Never tin the ends, I've found the normal Philips head works fine.
