So, is there some standard for how a click track is generated?  Does it need something on the one like one of the fancier old mechanical metronomes had with a bell or ?

Whatever you're comfortable playing along with.  I use Cakewalk Sonar which allows me to emphasize the first beat by making it louder than the other beats.  It also allows me to change which sounds the metronome uses.  I generally only use the metronome long enough to get a first track recorded (guitar or piano), then I mute it.
