Just tie the positive and negative leads of the new cord into the AC outlet on the amp, or wherever the original 2-prong cord connected. Then the earth ground connects to the chassis. This can be done by soldering it to the point where the power transformer is grounded to the chassis at one of the lugs. The ground switch and "death cap" underneath it do not serve any purpose once the 3-prong cord is installed so they can be removed from the circuit and the hole in the cassis can be used for a mod (master volume in my case).
