It's the song that plays in "The Royal Tenenbaums" when Luke Wilson attempts suicide. And for those who don't know, Elliot Smith killed himself by stabbing himself in the heart/chest. Its about the pain he felt and how alone he is in his addiction, and how he never wanted to feel that way again, but obviously he did many many times.
