Any one know the chords to Gavin DeGraw's "I Don't Want To Be"

Yeah ..... it goes into grungy guitar thing ......

 

|4/4 Gm | Gm Gsus4 | Gm | Gm Gsus4 |

 

| F { { F/C Bb | F7 { { F/C Bb | F7 { { F/C Bb |

 

|6/4 Eb / F / / / | 4/4 Ab / Eb / | Bb / Cm Bb/D |

 

...... where / = quarter note .... and { = quarter note rest.

 

The last two quavers of bar #4 are actually F/C Bb which leads into the stops on the F chord in bar #5. ( F/C = F chord with a C bass note. Bb/D = Bb chord with a D bass note ).

p.s. any suggestions for notating a 1/4 note rest are more than welcome.
