Or do you have another pot/switch for doing the split? Either way, it would be easy do both with one push pull. Wire the left half like the above schematic (assuming red is split and white is both coils - reverse if the color code doesn't match). Connect the center (out) from the left side to the center for the right side and to the neck position on the 5 way switch. Then connect the top terminal on the right side to the output jack.
