Nashville tele wiring question

I'm adding a middle pickup and a 5 way switch to my tele. I basically only want the middle pickup for the 2 and 4 out of phase positions. My question is: how do I wire the switch so the middle position on the switch is neck and bridge pickup??

Basically: position 1: bridge

position 2: bridge and middle out of phase

position 3: bridge and neck

position 4: neck and bridge out of phase

position 5: neck

Little hard to focus on but it will get you were you want to go.

That is exactly what Im looking for but what kind of switch is that?? Does it have more lugs than normal?? How would that translate to a typical 5-way switch with lugs on either side???

that would be a no go.. you need to get a 'super-5'
