It's a 12-bar blues, of course, so arrange those 3 lines accordingly. The intro is the G#m7 riff twice (break on last bar).

The (6) in brackets means this note is not always there - he's fretting it, but not always hitting it. (There's a similar vagueness about the other riffs, but it's quite intuitive once you have the shapes.)

The initial (9) is an approximate starting place for the slide.

... yes, I've just noticed the chord symbols and beat count isn't working... try this:

       G#m7------||-----------------------|-----------------------|------||-----------------------|-----------------------|------||------------------4----|-----------------------|------||------------4-----4----|------------4--4--6--4-|  x 2(9)\--||---------4--6----(6)---|---------4--6--6-------|------||4----------------------|4----------------------|       1  .  2  .  3  .  4  .  1  .  2  .  3  .  4  ..C#(m)9                                          (G#m7)|-----------------------|-----------------------||------------4-----4----|-----------------------||------------4-----4----|------------4--4--6--4-||---------4--------6----|---------4--6----------|  |4----------------------|4----------------------||-----------------------|-----------------------|1  .  2  .  3  .  4  .  1  .  2  .  3  .  4  ..D#(m)9                  C#(m)9                   (G#m7)|-----------------------|-----------------------||------------------6----|-----------------------||------------6-----6----|------------4--4--6--4-||---------6--8-----8----|---------4--6----------||6----------------------|4----------------------||-----------------------|-----------------------|1  .  2  .  3  .  4  .  1  .  2  .  3  .  4  .

The stock reference source is the Beatles, because their work is basically a repository of all the popular music that came before them (they'd listened to it all, pretty much), plus a few innovations; and all following pop/rock songwriters were influenced by them. There's very few good changes that you won't find somewhere in the Beatles' music.
