It's done all the time. Usual proceedure is to use a hair drier to remove the old one, use the old one as a pattern to make a new one, attach it with double sticky tape (some have the tape already applied). Uase naptha to remove any glue from the origianl one. I'm pretty sure Frank Ford show how on Frets.com, Bryan Kimsey has a wonderful gallery of materials that he uses - I had him replace the black one on my D-18 with a nice tortise looking material.

The wood under the p/g will be lighter colored so the new one has to be as large as the old (or do it immediately after you get the git) or you will have a "tan line".

Old Martins are a different matter - the p/g's were put on before the finish was applied and are harder to get off. They should be replaced to prevent (or fix) the typical top crack.
