Ideas for keeping the sustain pedal from sliding around? (hard wood floor)

Used to have that trouble alot... a simple, no-stick solution was to simply rotate the pedal 90 degrees, such that the 'back' is on the right side, and the pedal is on the left, perpendicular to how it would normally sit.

 

It works better if sort of pinned against your stand, but amazingly, just the weight shift seems to hold it in place better, and your foot doesn't know the difference.

 

Haven't had that problem since.

Double-stick carpet tape is my recommendation. That stuff is thin (so it won't raise the height significantly), but it is incredibly strong. And it's cheap as dirt. Home Depot or Lowe's will have it. And the best part? If you put in on properly, no one will know that its stuck to the floor until they TRY to move it. But, trust me, it won't move. :-)
