I think it's clear from the fact that the staple diet in American traditional cuisine (which I'm referring to things like the Hamburger, or the New York Pizza) aren't very nutritional. They are mostly carbs and protein. There are the frequent Caesar salads, but really most people that you see in those diners never order one.

One good thing about food addiction: it doesn't make one stoned or drunk. A brain can function fully and a person can be BRILLIANT yet still struggle with their weight. Plus, I've never met ONE fat person who didn't know they were fat and didn't want to be thin.
Pisses me off the poorest of people are able to shop at those stores using public assistance cards and buy such products that just make them more dependent in long term. Who thought that was a good idea?

You're right that it's about burning more calories than you take in.
It's also about knowing what you're eating. People don't understand that a fast food burger has astronomically more junk calories in it and less nutrients than a burger you make at home from the same ingredients purchased from locally grown, grass fed beef and a whole grain bun and whatnot. Compare the fries with similar weight of a baked potato with sensible toppings and again, astronomically more calories and much fewer nutrients.
The cost of 1000 calories in a grocery store for healthy food is much higher than the same 1000 calories of junk food.

I'd be more inclined to guess they live in Hawaii as I've lived in both Japan, Hawaii, and Okinawa and I'm betting they're living in Hawaii. The daughter definitely has some Pacific Islander in her and they definitely look like they've been eating Hawaiian food more than traditional Japanese fare.
I agree that in Japan "family style" is fairly common but portions and what they eat is much different than what you're seeing in that pic. Typically what I saw lots of was rice, veggies cooked up in a Wok, something like Chicken Katsu, etc.

I know that's a terrible thing to say, but he was making a point to me that you never saw an overweight survivor.
The other factors MAY include a medical reason, although that's a low percentage of the whole. But the "uninformed" excuse just does not cut it any longer. Most, if not all foods are labeled.
The problem is a lazy and "I want it now" attitude with society now. Couple that with a fast food restaurant on every corner and you get a fat nation.

It really is amazing how little people know about food. I don't know why we don't teach this in school. Glycemic index is something most people have never heard of but it's one of the most important things to understand about nutrition, especially if you are trying to lose weight.
Basically, glycemic index refers to how easily the body can convert a food to sugar. The easier a food is converted to sugar, the more readily it is stored as fat. Simple carbohydrates (sugary foods, refined flours) and fats are most easily converted to sugar. A steak may be 600 calories but your body can use those calories more slowly and efficiently than it could from a 600 calorie donut.

Sugar in particular seems to have an addictive property. When we eat processed sugar in particular, our insulin goes out of whack, creating a roller coaster effect on hunger. Instead of feeling satisfied, we get hungry for more sweets about half an hour after the first round. Food manufacturers know the power of sugar and put it in many foods where it never has been before. We don't "taste" it there, but its effect is the same.

I've noticed that all the women are ugly in red states as well.
